With PacificSource exiting Montana’s ACA Marketplace at the end of 2026, tens of thousands of Montana residents will be shopping for new health insurance for the first time in years.

When PacificSource exits on December 31, 2026, the market narrows to two carriers:

  • Blue Cross Blue Shield of Montana (BCBS MT) — Part of Health Care Service Corporation (HCSC), the largest nonprofit health insurer in the U.S. Offers both PPO and POS plan types.
  • Mountain Health CO-OP — A Montana-born, member-led nonprofit cooperative and one of only three ACA-created CO-OPs still operating in the country. Offers PPO plans.

Both are reputable, established carriers. The right choice depends on your health needs, location, budget, and which doctors you want to keep.

Blue Cross Blue Shield of Montana — What You Need to Know

BCBS MT is the dominant carrier in Montana’s individual market. Their plans offer robust provider networks across both urban areas like Billings, Missoula, and Kalispell, and Montana’s more rural communities.

Mountain Health CO-OP is a member-governed nonprofit — the people it covers have a voice in how it’s run. It offers PPO plans across Montana.

  • Average monthly premium: ~$662
  • Average deductible: ~$4,683–$5,800 (varies by plan)
  • Average out-of-pocket maximum: ~$5,800–$6,886

Mountain Health CO-OP often offers the most affordable monthly premiums in Montana. However, their higher deductibles mean more out-of-pocket costs before coverage kicks in — a trade-off that works best for generally healthy individuals who rarely need care beyond preventive services.

The key takeaway: Mountain Health CO-OP wins on monthly premium in most age brackets, but BCBS MT’s significantly lower deductible delivers better value if you actually use your insurance. For a 40-year-old who reaches their deductible, BCBS MT’s $2,568 vs. Mountain Health CO-OP’s $4,683 means over $2,100 in out-of-pocket savings — more than offsetting any premium difference.

Understanding Metal Tiers

Both carriers offer plans across the four ACA metal tiers:

  • Bronze: Lowest monthly premium, highest out-of-pocket costs. Best if you want a safety net but rarely use healthcare.
  • Silver: Middle ground. The only tier eligible for Cost-Sharing Reductions (CSRs), which can dramatically lower your deductible if your income qualifies.
